SUN SMART POLICY
Western District Health Service supports the Cancer Council Sun Smart Policy; slip on clothing; slop on sunscreen; slap on a hat; seek shade; slide on some sunglasses. Children are required to have their own sunscreen and hat

CANCELLATION OF ACTIVITIES
Western District Health Services reserves the right to cancel any scheduled activity due to poor weather conditions orinsufficient enrolments. As much notice as possible will be given. In such circumstances, a full refund will be provided.

FOOD AND DRINK POLICY
Please ensure that children bring food which does not require refrigeration or heating. Under no circumstance are energy drinksto be consumed or purchased. Children are required to bring a drink bottle with water.

Please note: given the location of activity, food options are limited. If your child has specific dietary requirements pleasemake prior arrangements with coordinator or, alternatively, ensure your child has sufficient amount of food for the day.

DEPARTURE LOCATION
All activities will depart from and return to the France Hewett Community Centre
2 Roberts Street
Hamilton VIC 3300.
Unless otherwise stated.

Please ensure your child is there 10 minutes prior to activity departure and collected at the time outlined in the program.
Participants not collected on time will be taken to Hamilton Police station for collection.

CLOTHING AND FOOTWEAR POLICY
Participants must wear appropriate clothing and footwear; no thongs. Personal hygiene is essential.

BEHAVIOUR AND CONDUCT POLICY
Participants demonstrating unsuitable behaviour will be removed from the program and may be denied future access. Nominated guardian will be notified to collect child.

Unsuitable behaviour includes:
> Refusing to follow instruction about conduct;
> Disrupting or affecting the enjoyment and participation of other participants;
> Affecting the health, safety and/or welfare of other participants or staff;
> Causing or likely to cause damage of property
> Likely to bring the program into disrepute > Bullying and harassment of other participants or staff
> Use of drugs or alcohol

REFUND AND CANCELLATION POLICY
Only cancellations made a minimum of five working days prior to activity will be eligible for a refund.

SUPERVISION Activities are supervised by WDHS staff, minimum of two staff per activity or ratio 1:10.
SPENDING MONEY

Spending money is optional.
It is up to the individual child to be responsible for their money.

GASTROENTERITIS AND COMMUNICABLE DISEASES
Communicable Diseases include: gastro, chicken pox, conjunctivitis, influenza, measles and shingles. Please note if your child has any of the listed communicable diseases they should not attend any activity delivered by WDHS. If your child is identified with a potential communicable disease a parent or guardian will be required to pick your child up from activity location, or alternate arrangements will be made at your cost.

WAIVER FORMS
Particular activities require Parents/guardians to complete waiver forms on behalf of the child attending the program.

PARTICIPANTS MAKING THEIR OWN WAY HOME
A written consent from parents and/or guardians is required for participants to make their own way home. To inform program leaders of these arrangements please attach a note onto your child’s registration form.
